Changing your Name

Your married, back from your honeymoon, your new home is in order and all the thank you cards have been sent but there is one more task is left.

 

Changing your name.

 

Here is a list of most of the common identifications and policies that you will typically need to change.

 

There may be more based on your personal needs.

  • Drivers License. Most states require you to change it with in a specific time frame. Don't get caught and end up with a ticket.

  • Social Security Card.  This also requires a limited time to make the change. Click here for the application form http://www.ssa.gov/ssnumber/ss5.htm

  • Bank and Savings and Loan Accounts.

  • Investment Accounts

  • Credit Cards

  • Forwarding Address if you've moved

  • Auto Registrations

  • All you "Membership" services. Fitness Clubs, Spas, etc.

  • Health Insurance.

  • 401K programs

  • Property Titles

  • Mortgages

  • Voters Registration Cards

  • Passports and Visas

  • Wills

  • Insurance Policy

  • Medical and Dental

  • Work related Policies

If there are children, yours and his, make sure your schools are informed of the marriage and that each of you are on file for pick-up and medical consent.  This will also apply to all the children's medical providers.  Do it now and not when your in a crisis.
